Key Contract Terms-admission

When individuals move into the home they have one-month built into their occupancy agreement to decide whether "This is the place for them". This first month then provides an opportunity for staff to get to know the individual resident and their family, and to identify their needs and preferred way of living, e.g. the time they like to go to bed and get up in the mornings.

During this period the person's care and support requirements are also assessed and discussed and developed into an agreed plan. This will include discussions and assessments of any risks to which the resident or staff may be exposed to. It also includes discussion, with the agreement of the resident with any relative or representatives who may be involved in the person's care needs and plans. The aim is achieve a plan of care with which everyone involved is happy.
 

Care-plan

The Care-plan for individual residents provides the basis of which St. Michaels care service is delivered. Each individual plan includes a description of their preferred daily routine, their likes and dislikes in relation to a description to food and any specific dietary requirements. It also includes their choice of respect to how they like to be addressed (e.g. Mr/Mrs) as well as how they interpret the meaning of dignity, respect, and privacy. We find that it is particularly important to find this out prior to any intimate personal contact that staff may need to carry out.

Each resident has a risk assessment as part of their care and any risk management plan.

The care plan also includes details of eating and drinking, personal hygiene/dressing, personal toilet, mobilisation, physical health, mental health, communication, socialising, and sleeping. The resident, a relative, a member of staff and a social worker if needed, will gather the information on the care-plan.

The daily care plan is organised as a response to service users individual and combined needs.
